1. Describe the problem in your own words
- Where do you feel pain, stiffness, swelling or weakness?
- When did it begin? Was there a fall, injury or change in activity?
- Which activities have become difficult: walking, stairs, sleep, work or sport?
- What treatments have you tried, and what changed?
2. Bring the records you already have
- Previous scans and their written reports, if available.
- Operation notes, discharge summaries and earlier prescriptions.
- A current medication list, including supplements, and known allergies.
- Relevant health conditions and previous injuries.
Ask the appointment team if they need anything specific. Do not arrange new imaging or stop medicines merely to complete this checklist; discuss those decisions with your clinician.
3. Choose the questions that matter to you
- What is the likely cause of my symptoms? Is anything still uncertain?
- Would a test change the treatment decision?
- What non-surgical and surgical options are relevant to me?
- What are the likely benefits, limitations and risks?
- What may I do at work, at home and during exercise?
- What costs, follow-up appointments or rehabilitation should I plan for?
- Which symptoms should prompt urgent help, and who should I contact?
4. Confirm the right clinic

5. Write down the next step
Before leaving, check that you understand the agreed plan, follow-up timing and contact route for further questions. Ask for an explanation if an instruction is unclear.
